Johnny's underwear, worn proudly on the head. At battle start they spawn something into the fight — what exactly, one prefers not to think about. SpawnOnBattleStart effects provide an immediate numbers advantage before any action is taken, making this a strong opener for swarm or overwhelm strategies despite its deeply undignified appearance.

SpawnOnBattleStart fires before the first action of any combatant, placing a unit or entity on the field at the very opening of every fight. The spawned entity can act from turn 1, providing a unit that costs nothing to summon and appears reliably across the entire run. As a head item it occupies a different slot than Tina's Best Friend, meaning both can be stacked for two battle-start spawns per fight.

Johnny's Undies is excellent for swarm-oriented builds that want consistent battlefield presence from the first turn. Pair it with Tina's Best Friend (face slot) for two spawns at battle start, then add Leather Hide Mask or Doodie Mask for per-turn spawning. Over an entire run the accumulated extra units from this head piece represent significant combat advantage. Entirely viable on any build that doesn't have a better head armor option.
