The Pinworm is a parasite neck item that lodges in the host's digestive tract and drains vitality, applying STR -1 and CON -1 penalties. Its passive, SpawnThingOnDamage, causes new creatures to appear whenever the wearer takes damage — turning every hit against the wearer into a potential battlefield disruption.

SpawnThingOnDamage triggers each time the wearer takes a hit, spawning an entity into the encounter. Unlike the Botfly Larva's single death-trigger, this passive can activate repeatedly throughout a fight, progressively filling the battlefield with spawned creatures. The spawns may be hostile to enemies, neutral, or chaotic depending on the specific implementation — but in any case, taking damage becomes a tool rather than purely a liability.

The more damage the wearer takes, the more creatures spawn — making this item paradoxically strong on characters with high health who can absorb many hits. Tanks and brawlers who intentionally draw attacks can turn Pinworm into a persistent summon engine. The STR -1 and CON -1 penalties are modest and don't significantly diminish a durability-focused role. Avoid on squishy characters who will be eliminated quickly before generating meaningful spawn value.
