Rat Bezoar

A common trinket from the Rat set that grants the PoisonThorns passive, dealing poison damage back to any enemy that strikes your cat. A strong defensive trinket at common rarity — it essentially punishes melee attackers for free and pairs well with low-health or high-damage-received builds where the thorns trigger frequently. Part of the Rat set alongside Rat Heart and Rat Tail.

PoisonThorns fires a Poison hit back at the attacker every time you take a melee hit, turning every enemy attack into a two-way trade. The Poison DoT then continues ticking on the enemy after the hit, meaning each incoming attack does progressively more total damage to the attacker than to your cat. Against melee-heavy encounters or multiple melee attackers, Rat Bezoar can accumulate substantial passive poison across an entire wave.

Rat Bezoar is exceptional for common rarity — PoisonThorns as a passive effect is normally a stat you'd expect on uncommon armor, not a common trinket. It's one of the few common-tier items worth holding deep into a run if your build takes melee damage frequently. Pairs with AmplifyStatus to increase Poison severity, and with Prayer Card to double up the incoming-hit synergies.
