The fight demands perfect execution, as the margin for error shrinks dramatically compared to Hard. Confusion is still one of the best tools for turning her power against herself, and sustained DOT from Bleed and Poison helps manage the inflated health pool. A dedicated Tank and Cleric are non-negotiable at this difficulty. Make sure your party is bred for maximum stats and equipped with the best gear available.
