Unidentified Pills

Unidentified Pills are a sidequest trinket with a bitter side effect: carrying them permanently raises the adventure's difficulty by one level. They provide +8 Shield as some compensation. Despite the unknown label, their impact on the run is immediately felt. Complete the sidequest to earn Truck Stop Pills, which provide Brace and a unique mechanical identity without the difficulty penalty.

+8 Shield is a meaningful defensive bonus, but a permanent difficulty increase affects the entire run from the moment you pick up the pills. Enemies hit harder, encounter more complex formations, and have more health at higher difficulties, meaning the Shield bonus alone does not compensate for the elevated threat level. The net effect depends on how far into the run you acquire the pills — earlier acquisition means more fights at elevated difficulty.

Equip Unidentified Pills on a cat who benefits most from Shield and assign the difficulty cost to your strongest party member. The sooner you complete the sidequest, the sooner the permanent difficulty penalty ends and you earn Truck Stop Pills' clean +3 Brace bonus. Avoid picking up Unidentified Pills late in a run when the difficulty increase affects the most dangerous encounters.
