Meat Altar
Before {catname} is a large altar made of veins and flesh. It demands sacrifice!
Meat Altar is a quest event found in the Throbbing Domain, a hidden Act 1 area accessed through the Guillotina quest chain. A grotesque altar of veins and flesh demands a blood sacrifice. The event serves two distinct purposes depending on whether it is your first visit or a return trip. On your first visit, a cat equipped with Guillotina's Head must be sacrificed to complete the quest and permanently unlock the Throbbing Domain. The cat is killed, but the domain opens for all future runs. On return visits, the quest sacrifice option still appears but the domain is already unlocked, making the Luck-based sacrifice the primary decision.
The Luck-based sacrifice kills your cat regardless of outcome, but the rewards (or penalties) for the surviving party vary dramatically based on a four-tier system. The best outcome grants a full party heal, +2 to all seven main stats for every surviving cat, and 3 items from the Blood Altar pool, a collection of body-part-themed equipment including Heart, Liver, Intestines, Sacred Heart, Crimson Mask, Red Cap, Pound of Flesh, Leaky Brain, Stomach, Lost Soul, Snaggle Tooth, and Bone Marrow. The good outcome provides a full heal, +1 to all stats, and 2 items. The bad outcome gives nothing at all: the cat simply dies for no benefit. The worst outcome curses the entire surviving party, inflicting 1 random disorder on each cat (excluding birth defects, chapter-specific conditions, Glorg blessings, and Deja Vu variants from the disorder pool).
To reach the Meat Altar for the first time, you must complete the three-stage Guillotina quest. Defeat Guillotina 1 to receive the Throbbing Gristle (deliver it to the meaty wall in The Caves), defeat Guillotina 2 for the Putrid Leach (deliver to the artery in The Boneyard), and defeat Guillotina 3 for Guillotina's Head (equip on a sacrificial cat and enter the Throbbing Domain). The first sacrifice costs you a cat, so equip Guillotina's Head on your least valuable party member. After the domain is unlocked, the Throbbing Domain's layout is linear: Meat Altar, normal battle, event, hard battle, treasure, then the boss fight against The Throbbing King. For return visit sacrifices, only attempt the Luck option with high-Luck cats. The stat bonuses and Blood Altar items from a best-tier outcome are among the most generous rewards in the game, but a worst-tier result saddles your entire team with disorders. The Ignore option is always available if you do not want to gamble.
